Payment for Passport Renewal

Please review the information below to assist you with the reimbursement process of your passport renewal fees.

In our current Contract on page 12-8 you will find the language regarding the cost of passports and passport renewals.

Passports

The Company will reimburse a flight attendant for the cost of

passports, passport renewal, passport photos, visas, inoculations and the

charge for expediting service for passports and visas in those domiciles

where passports and visas may be required. Any other associated expenses

will be reviewed on an individual basis.

On Wings, under Useful Links on the Inflight Page, you will find all the information regarding the process of renewing your passport as well as the requirements needed for the reimbursement.

Please note, original receipts must be submitted and if payment was made with a check for any of these items, a copy of the check must be submitted.

Reimbursement of Passport Renewal, Replacement and Expedited

Processing Expenses

1. Submit the following items to Claims Administration:

-- Completed US Airways form A61 "Crew Expense Report"

-- Front face photocopy of passport / Visa, or copy of your DS-82

-- Original receipts for the cost of pictures and passport processing

-- Expedited service receipt and mail service receipts, if applicable

2. Lost, stolen or name change passports are not reimbursed.

3. Other associated expenses with renewing or replacing passports such as transportation, parking or meals are not reimbursed.
