Hotel Problem - Use this form to report any hotel or transportation problems to us or the company. Directed to your MEC Hotel Chair and US AIRWAYS.
Questions & Rumors - Use this form if you have a Contract Question or a Contract Rumor Question. Directed to your local LEC President.
Scheduling Form - Use this form for problems with Schedulers on the phone. Directed to your LEC President.
Grievance Form - This form is for input and an initial data collecting form to see if any further action should be taken or to gather data for an ongoing grievance. Directed to your LEC President and LEC Grievance Chair.
Reserve Questions - Use this form for any Reserve Questions. Directed to your LEC Reserve Chair.
Cabin Air & Safety Form - Use this form any Cabin Air or Safety problems. Directed to your LEC President and LEC Safety Chair. Fill this out when your are also filling out the CSR below.
Save the Conversation Form - Use this form for recording your conversations with Scheduling as a backup. This form should be used while you are online and have scheduling with you on the phone. To be used in conjunction with having the "Contract Online" open as well. Directed to the Web Site Administrator.
