Dear fellow Council 82 Flight Attendants,

Subject: IMPORTANT REMINDER TO ALL 

You should have received a large envelope from USAirways in the recent days requesting your submission of important information in regard to the dependents covered under your benefits plan.

PLEASE OPEN AND COMPLETE THE FORM AND RETURN as instructed in the letter. FAILURE TO COMPLETE THIS FORM WOULD DISQUALIFY THEIR RESPECTIVE DEPENDENTS FOR ELIGIBILITY FOR BENEFITS. 

All dependents' eligibility must be verified to ensure continued coverage. 
This is a very busy month for all of us; however, keep in mind this process must be completed by December 28th.
